The Foster Care Case Manager-Master’s position is responsible for providing case management services, clinical oversight and crisis management, train, assists with home studies, assists with adoption cases, on-call, assists with referrals for the agency, as well as related to foster care youth placement as outlined in the essential functions listed below.

Essential Functions (include):

  • Shall assume the case work responsibilities for an assigned case load of youth in continuum of care.
  • Participate in the evaluation of referrals for continuum care and in the appropriateness of placing family foster homes.
  • Conduct the intake and assure the smooth transition of a foster youth into a foster home.
  • Participate in the planning and coordination of services for the youth while they reside in a family foster home.
  • Meet with the youth as determined by DCS policy and be available for crisis and/or emergency
  • situations. This position may be part of ratio during business hours as needed for youth/staff ratio compliance and in ratio for emergency/crisis situations as assigned by management
  • Coordinate appointments for the youth and ensure transportation and supervision is always available.
  • Assist in setting limits and dealing with consequences.
  • Participate with DCS on all planning and visitation requirements.
  • Oversee school admission and on-going attendance, as well as, intervention for problematic situations.
  • Bring case materials to treatment team meetings and be prepared to discuss caseload with supervisors.
  • Monitor clothing, allowance, visitation, medication, medical appointments, etc., for the youth in family foster care.
  • Maintain accurate and timely written records regarding interaction with or services provided to the youth in family foster care.
  • “On call” 24/7, as assigned, on a rotational basis. As well as back up for agency placements.
  • Work with program manager and provide clinical oversight as well as crisis management to clients and foster families.
  • Work with program manager to provide trainings, assist with home studies and serve as on call in program manager’s absence.
  • Assist with recruitment of foster parents.
  • Ensure that all required, updated personnel documentation is submitted to HR in a timely manner, and that all Agency training is completed prior to deadlines.
  • Perform other duties as assigned by supervisor.


Job Qualifications:

Master’s degree in social sciences, Master’s required. Experience working with children and families strongly desired, particularly in a foster care environment.

 

Physical Requirements (BFOQ):

  • Ability to participate in TCI physical restraint training as required (BFOQ)
  • Ability to verbally communicate clearly in English
  • Adequate sight and auditory abilities
  • Sit or stand for extended periods
  • Able to climb steps and/or hills (at residences) and walk as needed
  • Keyboarding abilities
  • Able to travel as required , mostly by automobile (valid TN driver’s license required)
  • Lift up to 40 pounds
  • Work extended hours as needed (>40 per week)
